About Project Transition Nashville

Project Transition supports adults ages 18 to 65 who live with mental health conditions, including people who also have substance use challenges, intellectual development disorders, or other physical health concerns. In Tennessee, the program provides residential or day treatment in apartment-style settings. The Nashville site is based at Haven Apartments, which was previously known as Woodbridge Apartments.

A key feature of Project Transition is its focus on building a therapeutic community within mental health care. The program uses the word “members” rather than “clients” or “patients,” with the goal of helping participants feel connected to a community and develop more purpose-driven lives that are integrated into everyday society. Members take part in the life of the apartment community, supporting a smoother return to the broader community and helping reduce stigma by reinforcing typical social expectations.

The center delivers dialectical behavior therapy (DBT) alongside an individualized, strengths-based approach in its residential mental health services. With a team of roughly 30 people, including certified peer and recovery specialists, staff provide structured daily support. Evidence-based interventions are adapted to align with each member’s goals. The staff are available from 7 a.m. to 9 p.m., and members can reach them after hours when emergency help is needed.

Each day includes skills workshops, medication management, therapies, and group meetings. Individual sessions, psychiatry appointments, and group therapy occur twice weekly. The remaining weekdays focus on additional community meetings, skills practice, goal setting, and recovery support through Alcoholics Anonymous (AA) or Narcotics Anonymous (NA) meetings. An onsite driver helps members travel from the apartment to off-site AA or NA meetings, as well as medical appointments.

Project Transition also provides systems of support (SOS) for children and adults whose behavioral health needs affect their community functioning. The program works with crisis teams and family support systems to reduce the likelihood of hospitalization and to help prevent crises.

Project Transition uses 13 criteria to track member progress and defines expectations for participation, including daytime visiting rights for family and friends when housemates agree, with no overnight stays. The program also uses partially unfurnished apartments, allowing members to bring their belongings.
