About Providence Center

On the Butler Hospital campus, The Providence Center Unity Health Home Team supports adults with serious mental health diagnoses, including schizophrenia, bipolar disorder, and depression. The center provides continuous, community-focused services for individuals who require more support than what standard outpatient appointments offer. With a coordinated team, clients can access help for both mental and physical well-being in one setting. The program combines multiple forms of treatment to promote stability and improve daily functioning. Participants may receive therapy, medication monitoring, treatment for substance use concerns, and assistance building practical life skills. Nurses, therapists, and case managers collaborate to design an individualized care plan based on each person’s needs and personal objectives. The Providence Center Unity Health Home Team also helps clients with practical needs such as finding housing, arranging transportation, and connecting to employment or education. Staff support appointment scheduling and help clients obtain the medications they need. The team is on call around the clock, offering guidance during difficult periods to help clients maintain progress and reduce the likelihood of hospitalizations.
