About Project Transition Reading

Project Transition supports adults aged 18 to 65 who have mental health concerns, including people with co-occurring conditions such as substance use, intellectual developmental disorders, or physical health issues. Treatment is provided through residential care in apartments.

A key feature of Project Transition is its focus on a therapeutic community approach. The center uses the word "members" rather than "clients" or "patients" to describe people receiving care. This is intended to help participants feel included in a community and work toward purposeful lives that are integrated into society. Members take part in day-to-day life within an apartment complex community, supporting a smoother return to the wider community. The program also seeks to reduce stigma and promote more typical social expectations.

Project Transition applies dialectical behavior therapy (DBT) within its residential programs, combining DBT with an individualized, strength-based approach. A team of around 30 staff members, including certified peer and recovery specialists, provides daily support and uses evidence-based interventions tailored to each member’s goals. Staff are available from 7 a.m. to 9 p.m., and members may reach out after hours when emergency help is needed.

Throughout the day, staff organize skills workshops, medication management, therapies, group meetings, and other activities. Individual meetings, psychiatry appointments, and group therapy sessions typically occur twice per week. The remaining days include community meetings, skills practice, goal setting, and Alcoholics Anonymous (AA) or Narcotics Anonymous (NA) meetings. An on-site driver can take members from the apartments to off-site AA or NA meetings and to medical appointments.

In addition, Project Transition provides systems of support (SOS) for children and adults whose behavioral health challenges affect their communities. The program collaborates with crisis teams and family support systems to lower the likelihood of hospitalizations and help prevent crises.

To track progress, Project Transition uses 13 criteria for members, including rules such as allowing family and friends to visit during the day if housemates agree, while visits cannot include overnight stays. The center also uses partially unfurnished apartments, and members are allowed to bring their belongings.
