SSM Health's Adult Day Services supports community members dealing with mental health challenges, serving older adults, individuals with chronic illnesses, and people with disabilities. The center offers a variety of specialized programs and services supervised by medical staff, delivering care designed around each client’s unique needs.
The wheelchair-accessible Adult Day Services site in Fond du Lac provides mental health care that includes group therapy and psychiatric services. It also offers day treatment, referred to as partial hospitalization (PHP).
Hours are Monday through Friday, 7:30 a.m. to 5:30 p.m. During and after treatment, clients can view care notes by signing in to MyChart, their online patient portal.
