About Aware Recovery Care Virginia

Aware Recovery Care Virginia uses a “Recover Where You Live” model for addiction treatment for adults 18 and older. Through its in-home addiction treatment (IHAT) program, clients receive a blend of in-person and remote support, including concierge-style help designed around individual circumstances. The center treats substance use using a biopsychosocial program that draws on proven therapies, medication-assisted treatment (MAT), individual counseling, family therapy, and coordinated services involving outside professionals when needed. The program’s approach is based on empathy, respect, and the idea that everyone should have a real chance to build long-term recovery. By focusing on each person’s specific needs and offering steady guidance at home, Aware Recovery Care aims to support clients on the path to a healthier, more meaningful, addiction-free life. Each client is assigned a care coordinator, such as a nurse or social worker, along with two client recovery advisors (CRAs) who provide peer support and help coordinate care throughout treatment. The team determines what additional outside services are needed and schedules appointments with a primary care provider (PCP), individual therapist, relevant medical specialists, and a family education facilitator. When clients already have these providers, Aware Recovery Care’s medical director works with them to coordinate ongoing care. The family education facilitator also educates and supports loved ones, helping them learn about addiction and recovery and how to heal together. Treatment continues on a monthly structure, with program length typically ranging from one month up to a year. The program combines home-based and virtual services and includes as many as 20 one-on-one virtual therapy sessions with an Internal Psychotherapist. In these sessions, clients work on stress management and improving self-esteem, self-compassion, and communication. They also identify unhelpful thinking patterns and learn strategies to change them. A typical routine at Aware Recovery Care includes an hour with a member of the care team. Depending on the day, clients may receive an in-person visit from one of the two CRAs or participate in a video session with their one-on-one therapist. Clients also meet monthly with their Care Coordinator. Throughout the program, the care team regularly communicates to ensure the plan stays aligned with clients’ evolving needs and goals. When it is time to transition out of treatment, Aware Recovery Care supports clients with a personalized discharge plan matched to their treatment history and home environment. Clients review the full discharge strategy with their clinical coordinator, including plans for continued support, progress toward established objectives, and follow-up appointments with licensed providers connected to their care journey.
