About Project Transition Wilmington

Project Transition provides care for adults aged 18 to 65 who are living with mental health conditions, including people with co-occurring issues such as substance use, intellectual development disorders, or physical health concerns. The center delivers residential treatment in apartment-style housing.

A key feature of Project Transition is its focus on a therapeutic community approach within mental health care. Instead of referring to program participants as “clients” or “patients,” Project Transition uses the term “members” to reinforce belonging and support members in building purposeful lives that are fully integrated into the community. Members take part in life within an apartment complex community located in a space Project Transition rents from Cypress Grove Apartments, helping them reconnect with the wider community. This approach is intended to reduce stigma and promote everyday social expectations.

The programs at Project Transition use dialectical behavior therapy (DBT) combined with an individualized, strengths-based perspective. The residential team is made up of about 30 staff members, including certified peer and recovery specialists, who provide daily support and use evidence-based interventions aligned with each member’s personal goals. Staff are available from 7 a.m. to 9 p.m., and members can also reach out after hours when emergency help is needed.

Throughout the day, staff facilitate skills workshops, medication management, therapies, group meetings, and other structured activities. Individual sessions, psychiatry appointments, and group therapy occur twice each week. The remaining days include community meetings, additional skills workshops, planning around goals, and support meetings such as Alcoholics Anonymous (AA) or Narcotics Anonymous (NA). An onsite driver helps members travel from the apartment to off-site AA and NA meetings, as well as to medical appointments.

In addition, Project Transition offers systems of support (SOS) for children and adults experiencing behavioral health concerns that affect their community. They collaborate with crisis teams and family support resources to reduce the likelihood of hospitalizations and help prevent emergencies.

Project Transition also outlines 13 criteria used to evaluate member progress and success. These include: family and friends may visit during the day if housemates agree, but overnight stays are not allowed. Project Transition uses partially unfurnished apartments, and members can bring personal items.
