About Sobriety Centers of New Hampshire - Lakes Outpatient

Sobriety Centers of New Hampshire provides an integrated addiction treatment pathway that includes detox, residential, and outpatient services for adults 18 and older. The Lakes Addiction Treatment Center offers flexible outpatient programming with different care intensities, supported by clinical professionals and personalized master treatment plans created by licensed and qualified addiction specialists.

Treatment is tailored to each person and follows a holistic model that combines individual counseling, group therapy, and family therapy, medication monitoring, life-skills development, educational sessions, and workshops based on 12 Step or SMART Recovery approaches. Along with nearby detox and residential care through Antrim House, Lakes also provides a partial hospitalization program (PHP), an intensive outpatient program (IOP), and a standard outpatient (OP) track that includes ongoing one-on-one therapy.

The PHP schedule runs up to 5 hours per day, as much as seven days per week, depending on the individualized plan. This level of care offers many of the same supports as inpatient treatment while allowing clients to live at home. PHP may be used after residential care as a step-down level, or it may serve as the starting point for treatment based on each client’s needs. PHP at Sobriety Centers includes routine drug testing, individual and group and family therapy, assistance with medical needs such as medication management and medication-assisted treatment (MAT), and career-related supports through an employer assistance program, among other services.

IOP is the next phase within their full continuum of care. It includes group therapy for about 12 hours per week along with weekly one-on-one sessions. IOP helps clients rebuild connections with family and community while still participating in normal daily routines. Intensive outpatient includes relapse-prevention and coping strategies, psychoeducation, peer support, and ongoing development of practical life skills.

At Lakes, clients begin with comprehensive assessments and work with the team to create individualized treatment plans. Programs combine evidence-based approaches, including several forms of cognitive behavioral therapy (CBT). The center also uses a holistic framework that may include activities such as yoga and meditation. Sobriety Centers further focuses on the roots of addiction and provides support for co-occurring mental health conditions, including anxiety, depression, trauma, and PTSD.

Sobriety Centers of New Hampshire also offers substantial aftercare. This may involve continued group therapy, access to community resources, vocational and housing assistance, medication management, support meetings, continuing care options, self-help groups, relapse prevention resources, and additional outpatient treatment as needed.

As the first New Hampshire facility to adopt a trauma-informed approach, Sobriety Centers emphasizes compassionate care aimed at addressing the underlying causes of substance use and compulsive addictive behaviors.

Sobriety Centers additionally offers a specialized program track for veterans dealing with substance misuse and mental health challenges. Services are designed for both active duty and retired military members and include support from highly trained addiction specialists to meet each veteran’s specific needs. Veterans can choose among detox, residential, and multiple outpatient options that include dual-diagnosis care at varying levels of intensity.
