Freedom Health is an outpatient rehabilitation center that delivers versatile, full-spectrum support through its partial hospitalization program (PHP), intensive outpatient program (IOP), and outpatient program (OP) for people dealing with dual-diagnosis conditions. The center’s caring team focuses on helping clients and their families create a stable base for long-term recovery.

At Freedom Health, the partial hospitalization program (PHP) represents the highest and most structured level of care. While it is typically less restrictive than residential treatment, it still provides intensive medical and psychological services, giving clients a strong support system without requiring a full residential commitment. The PHP schedule typically includes about 6 hours of treatment daily, 5 to 6 days per week, combining holistic practices with evidence-based behavioral approaches, along with wellness education, therapeutic groups, and recreational activities.

The intensive outpatient program (IOP) comes next and is designed as a step down from PHP. It uses similar treatment methods and holistic components but generally involves fewer hours, typically 15 to 20 hours per week. Clients may choose morning or evening sessions depending on their needs and availability.

Treatment begins with an initial bio-psycho-social evaluation so clinical staff can understand each person’s specific challenges and build a personalized plan. The overall approach supports physical, emotional, and spiritual recovery for individuals with addiction and co-occurring disorders. Programs may include process groups for discussing the roots of addiction, cognitive behavioral therapy (CBT) groups, and group personal training sessions that address exercise and nutrition as part of overall wellness. Additional services can include vocational and life skills support, legal assistance and case management when needed, family support and education, and psychiatric and medical care, including medication-assisted treatment (MAT). Freedom Health also offers complementary options such as chiropractic and massage therapy, yoga, mindfulness, and experiential therapies. Topics covered in group sessions may include addiction education, the fundamentals of recovery, recovery principles, relapse prevention, anger management, codependency, and psychoeducation.

Freedom Health’s outpatient program (OP) is the most flexible level of care. OP focuses on dual-diagnosis treatment through weekly individual therapy appointments, with the option to participate in a hybrid format using both in-person and virtual sessions. This program supports progress after treatment through fitness-focused goals, nutrition and healthy routines, time management and organizational skills, vocational training and job-search assistance, and additional holistic supports such as yoga and meditation.

Aftercare Support Freedom health provides aftercare as part of its ongoing recovery model. Services include monthly recovery outings, weekly group meetings supported by staff, follow-up calls and accountability check-ins, volunteer opportunities, and help connecting with recovery-based fellowships. The program may also offer periodic referrals and resources for additional external care.

Length of care and capacity The average treatment duration at Freedom Health is about 2.5 months. The center can support approximately 10 to 15 clients at a time, with a 5-to-1 ratio between clients and the clinical team.
