About CleanSlate Centers
CleanSlate Centers, based in Merrillville, Indiana, is an outpatient rehabilitation facility focused on behavioral health care and targeted treatment for addiction. The clinical team collaborates with each client to build individualized recovery plans and provides medication-assisted treatment (MAT) for adults dealing with alcohol and opioid addiction. MAT can help lessen cravings and support individuals through withdrawal, while the program also addresses accompanying mental health concerns such as anxiety, depression, and trauma.
At CleanSlate, licensed professionals deliver Medication-Assisted Treatment (MAT) for people experiencing physical dependence on alcohol or opioids. This treatment uses FDA-approved medications, including Suboxone and Vivitrol, to reduce withdrawal discomfort. For best outcomes, MAT is typically paired with counseling and mental health services. The center also offers a specialized track for pregnant women, with medical supervision to help safeguard the health of both mother and child.
CleanSlate provides a comprehensive and personalized approach. Treatment options may include MAT, individual therapy, group therapy, and family therapy. Aftercare planning includes help with housing, transportation, employment guidance, and ongoing support from certified recovery coaches. CleanSlate is CARF-accredited and works with Medicaid, Medicare, and most commercial insurance plans. The Merrillville site is open Monday, Tuesday, and Thursday from 8 AM to 4 PM, Wednesday from 8 AM to 7 PM, and Friday from 8 AM to 1 PM. New patients can also attend walk-in hours from 9:30 to 11:30 AM and 1:30 to 3:30 PM Monday through Thursday, and 9:30 AM to 12:30 PM on Friday.
