About CFHW Specialty Behavioral Health Servs
Clinica Family Health & Wellness was created by merging Clinica Family Health with Mental Health Partners. The center delivers coordinated care across physical, mental, and oral health for people of all ages, with programs specifically designed for older adults 65+ and young adults 15–29.
The Clinica St. Vrain Community Hub serves clients roughly 30 minutes southwest of Boulder. Its services focus on issues such as depression, anxiety, bipolar disorder, trauma, and substance use disorders. With a community-based model, the hub makes treatment more reachable and culturally appropriate, independent of someone’s ability to pay or immigration documentation status.
The center uses evidence-based approaches, including cognitive-behavioral therapy (CBT), trauma-focused therapy, medication management, and peer support. Clients can access both individual and group counseling, psychiatric care, and wellness programming. Genoa Healthcare supplies medications on site through the pharmacy located at the St. Vrain hub.
Clinica also runs a 24/7 walk-in crisis center that provides prompt assistance for urgent situations, including those who may be considering suicide or self-harm.
The Clinica St. Vrain Community Hub is located in Longmont near the Main Street & 6th Avenue bus stop. In-person services are offered Monday through Friday, from 8:00 a.m. to 5:00 p.m.
