About Evolve Teen Outpatient Treatment

Evolve San Jose is a comprehensive partial hospitalization program (PHP) and intensive outpatient program (IOP) designed to provide customized, solutions-oriented care for adolescents ages 12–17. The center employs master’s-level clinicians, interns, primary therapists, and experienced counselors and recovery coaches, creating a structured, supportive setting that promotes safety and healing. Its dual-diagnosis services focus on treating mental health conditions, substance use issues, and co-occurring disorders through evidence-based and skills-based approaches, including group therapy, family therapy, motivational interviewing (MI), cognitive behavioral therapy (CBT), and dialectical behavior therapy (DBT).

During intake, trained clinicians conduct a detailed assessment to develop an individualized treatment plan. Plans are reviewed and adjusted on an ongoing basis to match each teen’s evolving needs and goals.

Evolve provides care for a broad range of challenges, including anxiety, depression, self-harm or suicidal thoughts, post-traumatic stress disorder (PTSD) and trauma, bipolar and other mood disorders, gender dysphoria, disordered eating, borderline and other personality disorders, mood or behavioral dysregulation, obsessive-compulsive disorder (OCD), and psychosis.

For substance use treatment, the program supports teens dealing with alcohol, cannabis, opioids, prescription medications (including stimulants and sedatives), synthetic substances such as hallucinogens and stimulants, and tobacco-related concerns.

The dual-diagnosis program also serves teens who experience both mental health and substance use conditions at the same time, including those diagnosed with high-functioning autism spectrum disorder (ASD) when a co-occurring mental health or substance use issue is present.

Evolve San Jose offers two levels of care: a partial hospitalization program (PHP) and an intensive outpatient program (IOP). The PHP, also known as day treatment, is the most intensive option, with clients attending a full day of programming five days per week. The IOP is geared toward teens who live at home and attend school, offering about three hours of treatment three to four days each week. Both programs are suitable for teens beginning treatment and for those stepping down from residential care who still need structured support.

Alongside the most current evidence-based practices, the treatment approach may include experiential therapies. Art therapy can be used to promote relaxation, reduce stress, strengthen self-understanding, and help clients work through past experiences. Expressive modalities such as music and dance therapy may support mood, overall wellbeing, and body image, while drama therapy can enhance communication, strengthen relationship skills, and encourage empathy.

Clients and families receive clinical support through individual, group, and family therapy sessions led by a dedicated professional team. DBT is a central focus, with the program designed to follow DBT principles and include weekly DBT group therapy. CBT is also used to address depression, anxiety, substance use, and eating disorders by helping teens shift unhelpful thought patterns. Motivational interviewing (MI) helps clinicians connect with each teen’s current stage, clarify personal goals, and strengthen motivation for change. Mindfulness is incorporated throughout treatment to encourage awareness in the present moment and acceptance without judgment.

Additional strategies for substance use and co-occurring disorders include seeking safety, relapse prevention, anger management, meditation, and life skills. Family therapy and family support groups are also available to help families stay involved and work alongside the care team.

Evolve San Jose is Joint Commission and CARF accredited and accepts most major insurance plans.
