This outpatient community-based rehabilitation center uses a coordinated model that brings together primary care with mental health, substance use, and trauma-related support. When individuals with substance use needs enter the program, the intake team evaluates their circumstances to decide whether they should be placed in the intensive outpatient program (IOP) or the standard outpatient track.
